What you will do:

• Manage a dynamic caseload of residents with complex needs, applying your expertise in social services and community equivalent settings such as rapid response.
• Employ strong Moving and Handling skills to ensure safety and efficiency in practical applications.
• Assess and plan for major and minor home adaptations to facilitate independence and improve quality of life for those you support.
• Embrace a hybrid working model, splitting your week between three days in the office or on visits and remote work.

Qualifications / Requirements:

• Over two years of experience in social services or equivalent community settings.
• HCPC registered.
• Valid driving licence, as this role requires travel for assessments and visits.
