What you will do:

- Conduct comprehensive Occupational Therapy assessments within Adult Social Care settings.
- Review and manage care and support needs for service users.
- Assess and recommend equipment that champions independence.
- Evaluate and advise on major adaptations for individuals needing specific home modifications.
- Perform complex moving and handling assessments to ensure safety and effectiveness.
- Support residents to stay independent within their communities.
- Create person-centred intervention plans and collaborate with service users, carers, and multi-disciplinary professionals.
- Participate proactively in duty service responsibilities.
- Maintain accurate, thorough records and case documentation.

Requirements:

- Degree or Diploma in Occupational Therapy.
- HCPC registration is essential.
- Experience in Adult Social Care Occupational Therapy.
- Proficiency in conducting complex moving and handling assessments.
- Capability in assessing for major adaptations.
- Skill in recommending appropriate equipment.
- Aptitude for managing an independent caseload with strong assessment and clinical reasoning abilities.
- Exceptional communication and report-writing skills.
- Ability to efficiently participate in a duty rota.
- Experience in Local Authority settings.
- Previous work within Adult Social Care teams.
- Experience with complex community cases.
- Background in supporting interventions focused on independence.
