Occupational Therapist - Intermediate Care Team within Intermediate Care in North Leeds UK earning up to £30 per hour depending on experience on an hourly basis. As an Occupational Therapist in the Intermediate Care Team you will provide vital services under the guidance of a Team Manager while benefiting from the support of Senior Occupational Therapists.

What you will do:
- Deliver comprehensive Occupational Therapy services to adults children and young people as well as carers in line with organisational needs and statutory guidelines.
- Conduct thorough Occupational Therapy assessments utilizing a strength-based personalized approach to develop and implement goal-oriented outcome plans.
- Work collaboratively with support workers to ensure individuals achieve their outcomes and regularly review their progress.
- Lead discharge planning for individuals returning to their homes.
- Apply moving and handling skills to assess and identify solutions for reducing risks in line with the personal outcomes of disabled individuals and their families.
- Actively participate as a part of the Leeds Moving and Handling initiative to ensure optimised and safe care practices are in place.
