What you will do

  • Screen and prioritise complex referrals received through the Contact Centre to ensure immediate needs are met efficiently.
  • Conduct person-centred, strength-based assessments to formulate effective support plans.
  • Provide expert guidance to Enabling Independence Officers and Trusted Assessors to promote excellent service delivery.
  • Work collaboratively within a multidisciplinary duty team to deliver comprehensive support to the community.
  • Recommend and implement suitable equipment and adaptations, facilitating residents' independence.
  • Complete proportional assessments, effectively signposting residents to the appropriate services.
  • Demonstrate safe and effective use of recommended equipment to clients and their families.
  • Assess eligibility and manage processes for major adaptations, including structural changes and lifts.
  • Present cases to Quality Assurance Panels as required to maintain high standards of client care.
  • Liaise efficiently with Housing, Social Care, and partner agencies, ensuring coordinated support for residents.
  • Assist in finding suitable rehousing solutions for clients when necessary.
