What you will do:
- Complete Occupational Therapy assessments for adults within the community
- Assess functional ability and promote independence and wellbeing
- Recommend and arrange specialist equipment
- Assess for and recommend minor and major adaptations
- Manage a caseload of residents with complex needs
- Develop person-centred intervention plans
- Work collaboratively with Health and Social Care partners
- Support residents to achieve positive outcomes and maintain independence
- Maintain accurate records, reports, and case documentation
- Contribute to pathway development and service improvement initiatives
Requirements:
- Qualified Occupational Therapist with HCPC registration
- Experience in Adult Social Care Occupational Therapy
- Skilled in assessing for equipment provision and adaptations
- Strong assessment and clinical reasoning skills
- Experience managing a caseload independently within a multidisciplinary team
- Excellent communication and report-writing skills
- Ability to thrive in a fast-paced environment
Desirable Requirements:
- Local Authority experience
- Experience within integrated health and social care services
- Experience contributing to service development
- Experience managing complex community cases
