What you will do:

• Conduct community assessments in a local authority setting, identifying needs and recommending both standard and specialist equipment.

• Plan and facilitate equipment and minor adaptations to enhance safety and functionality within clients' homes.

• Collaboratively work with clients, families, Social Workers, and Assessment and Review Officers to create individualised support plans.

• Maintain thorough and accurate records in line with professional and legal standards.
