What you will do:
- Support, provide, and develop a responsive, proactive, and patient-focused pharmacy service to the Renal Unit
- Provide an accurate and timely medicine management service to patients served by the South West Thames Renal and Transplant Unit and its satellites
- Assist in the provision of ward-based pharmacy services to the renal unit
- Optimise drug management by taking drug histories, assessing patients’ own drugs at admission for use during their hospital stay, promoting one-stop dispensing, and reducing unnecessary dispensing of medication during their stay and on discharge
- Resolve and/or appropriately refer interventions back to the clinical pharmacist or liaise with other members of the MDT within scope and experience
- Record details of medication history, patient’s own supply, and any other relevant information on the electronic in-patient prescription chart in accordance with pharmacy procedures
- Document interventions and discrepancies related to prescribing of medicines on the electronic patient records
- Encourage and support compliance with the local formulary and prescribing policies and guidelines
Requirements of the Pharmacy Technician:
- GPHC Registration
- Previous experience within a UK based Pharmacy setting
- NVQ or equivalent – Pharmacy Technician qualification
