What you will do:

  • Provide tailored support and intervention to individuals with complex needs, including those with substance misuse issues, mental health challenges, health concerns, and safeguarding issues.
  • Manage a diverse caseload, using your expert knowledge to deliver harm reduction and recovery models.
  • Conduct assessments, develop personalised care plans, and coordinate with a multidisciplinary team to ensure comprehensive support for each service user.
  • Facilitate group sessions and one-on-one interventions aimed at promoting recovery and resilience in clients.
  • Work collaboratively with internal and external agencies to provide holistic, client-centered care.
  • Maintain accurate records and documentation, ensuring compliance with organisational policies and procedures.

Requirements:

  • An up-to-date DBS check on the update service.
  • Demonstrable experience in substance misuse, specifically with drugs and alcohol.
  • Strong skills in caseload management and keyworking.
  • A solid understanding of harm reduction and recovery models.
