What You Will Do:

• Conduct independent assessments, develop treatment plans, and manage discharge planning for community rehabilitation patients.

• Apply skills in equipment reasoning and ordering to support patient care.

• Utilise recognised outcome measures to monitor and evaluate patient progress effectively.

• Facilitate family meetings to ensure comprehensive care and support.

• Travel to various community locations to deliver care and support; a driving licence is required.

HCPC registration and relevant post-qualification experience as an Occupational Therapist are essential, ideally within community and rehabilitation settings.
