What you will do:

Conduct comprehensive occupational therapy assessments and devise tailored rehabilitation programmes.
Manage a neurological rehabilitation caseload, ensuring that each patient receives personalised care.
Drive patient-centred stroke rehabilitation pathways, supporting functional recovery and independence.
Facilitate seamless discharge planning and community transitions, working closely with external services.
Maintain meticulous clinical records, ensuring all documentation is complete and accurate.
Collaborate efficiently with multidisciplinary teams to deliver holistic patient care.
Engage in ongoing personal and professional development, keeping your skills and knowledge at the forefront of stroke rehabilitation.

Requirements:

Possess a Degree or Diploma in Occupational Therapy and hold valid HCPC registration.
Demonstrable experience in stroke and neurological rehabilitation within the NHS in the UK.
Strong assessment and clinical reasoning skills, with the ability to lead an independent caseload.
Familiarity with community rehabilitation and stroke services is desirable, alongside experience in handling complex neurological cases.
