What you will do:

• Provide comprehensive pharmaceutical care to patients on surgical wards, ensuring the safe and effective use of medication.
• Perform accurate medication history checks and provide counselling to patients regarding their prescribed medications.
• Collaborate closely with the multidisciplinary team to develop individualised care plans.
• Ensure accurate and efficient dispensing of medications to enhance patient outcomes.
• Maintain up-to-date and thorough documentation according to department protocols.
• Participate in ward rounds to provide pharmaceutical expertise and immediate therapeutic advice.

Requirements:

• HCPC registration.
• Minimum of 2 years' experience working in surgical and medical adult wards.
• Strong knowledge of pharmaceutical care within acute hospital settings.
• Excellent communication and multidisciplinary team-working skills.
• Driving licence preferred but not essential.
