Key Responsibilities / What You Will Do

  • Deliver high‑quality physiotherapy interventions within a community setting, with a focus on admission avoidance.
  • Complete comprehensive assessments and develop individualised, goal‑oriented treatment plans.
  • Work closely with A&E departments to ensure seamless transitions and continuity of patient care.
  • Collaborate with multidisciplinary colleagues across health and social care services.
  • Attend team meetings and contribute to service development and quality improvement initiatives.
  • Maintain accurate and timely clinical documentation in line with HCPC and local standards.
  • Travel independently to patient homes and community settings as part of your caseload.

Requirements / Candidate Criteria

Essential

  • HCPC‑registered Physiotherapist.
  • Valid driving licence for community travel.
  • Previous experience in A&E and community physiotherapy roles.
  • Strong assessment, decision‑making, and clinical reasoning skills.
  • Ability to work independently across varied community environments.

Desirable

  • Experience in admission avoidance or urgent community response services.
  • Familiarity with integrated community care pathways.
