This is a role tailored for those who thrive on challenge and variety. Experience in Young Adult Mental Health Services (YAMHS) as well as crisis work is pivotal here. You will be covering duty specifically on three days a week, supporting discharges, screening, welfare calls, information gathering, and interface meetings, all of which help the throughput of the team. Confident management of local systems is a key requirement, including how to refer to the Home Treatment Team, Mental Health Act processes, and steps down to wellbeing. You must shine in working independently with minimal supervision, offering thorough face-to-face assessments and delivering insightful formulations.

- Conduct comprehensive assessments and develop tailored treatment plans

- Facilitate discharge planning and coordinate community resources
- Maintain accurate documentation and case records
To be successful in this role, you must be HCPC registered and hold a valid UK driving licence due to the nature of travel required for this position. The role demands proactive problem-solving skills and the ability to adapt to rapidly changing environments.
