What you will do

Manage a caseload of clients with complex mental health needs, providing comprehensive assessments, care planning, and interventions.
Work collaboratively with multidisciplinary teams, including psychiatrists, social workers, and support staff, to deliver coordinated care.
Facilitate group sessions and individual therapy to promote mental health recovery and independence.
Conduct home visits to provide community-based support, ensuring continuity of care for individuals in their own environment.
Maintain thorough records of patient progress and report findings to senior staff as necessary.
Support individuals in crisis, using de-escalation techniques and providing rapid intervention when required.
