Band 6 Community Care Coordinator

Location: Croydon, UK

Pay Rate: £27 per hour

Availability: ASAP for 3 months, Monday to Friday, 9 am to 5 pm (37.5 hours per week)

Join the vibrant Croydon community as a Band 6 Community Care Coordinator, where you'll earn £27 hourly working with the South London & Maudsley NHS Foundation Trust. This role is available for an initial 3-month period starting ASAP at Jeanette Wallace House. If you are a Registered Nurse (Mental Health), a qualified Social Worker, or have a background in Occupational Therapy, this is a fantastic opportunity to immerse yourself in community healthcare.

What you will do:

  • Coordinate and deliver outstanding mental health care within the Croydon Assessment and Liaison team.
  • Conduct comprehensive assessments with clients to develop personalised care plans.
  • Support individuals with complex mental health conditions, utilising evidence-based interventions.
  • Collaborate with multidisciplinary teams to ensure seamless integration of services across community and inpatient settings.
  • Manage a diverse caseload, using your knowledge from community mental health or acute inpatient environments.


Requirements:

  • Registered Nurse (Mental Health) or equivalent professional registration in Social Work or Occupational Therapy.
  • Demonstrable evidence of continuous professional development.
  • Proven experience in community mental health settings or acute inpatient environments.
  • Strong understanding of interventions for complex mental health conditions.
