What you will do:

• Assess and treat patients within an acute hospital environment, ensuring the highest quality of care delivery.
• Manage acute Physiotherapy or Occupational Therapy caseloads efficiently.
• Complete functional assessments and develop comprehensive treatment plans tailored to individual patient needs.
• Support safe discharge planning and patient flow, ensuring a seamless transition and continuity of care.
• Work closely with multidisciplinary teams, fostering collaboration and shared learning.
• Deliver evidence-based rehabilitation interventions designed to maximise patient outcomes.
• Maintain accurate clinical records and documentation, upholding professional standards.
• Participate in ward-based patient management and reviews, contributing to team discussions and patient-centered care.

Requirements:

• HCPC registration
• Acute hospital experience
• NHS experience
• Experience managing acute inpatient caseloads
• Strong assessment and clinical reasoning skills
• Ability to work independently and within multidisciplinary teams
• Excellent communication and organisational skills

Desirable Requirements:

• Previous Band 6 Physiotherapy experience
• Previous Band 6 Occupational Therapy experience
• Experience within integrated discharge pathways
