What you will do:

• Provide high-quality physical and emotional care for adults with learning disabilities, including personal care, medication management, and household support.

• Assist service users in their own homes and extra-care housing using the Active Support Model to foster independence.

• Work safely and confidently during lone outreach visits, adhering to organisational policies.

• Collaborate with colleagues and senior staff to uphold service values and quality standards.

• Act as a co-key worker, guiding and supporting service users to access community opportunities and develop independence.
