What you will do:

Undertake Care Act 2014 assessments, analysing needs and eligibility for support to design bespoke care plans for individuals.
Manage a diverse caseload of adults with mental health needs, including complex and high-risk cases.
Conduct Mental Capacity Assessments and make Best Interest decisions in accordance with the Mental Capacity Act 2005.
Participate in and lead Care Programme Approach (CPA) processes, ensuring holistic care planning.

Qualifications:

Degree in Social Work (BA/MA or equivalent).
Evidence of continued professional development.

Registration & Compliance:

Registered with Social Work England.
Knowledge of relevant legislation and frameworks, including the Care Act 2014, Mental Health Act 1983/2007, and the Mental Capacity Act 2005.
Enhanced DBS clearance required.
