What You Will Do

• Carry out timely and comprehensive care management assessments for service users and their carers, identifying and prioritising desired outcomes.

• Promote independence and recovery by supporting enablement and connecting individuals to appropriate community-based services.

• Work collaboratively within multi-agency teams to deliver coordinated health and social care services.

• Partner with Support Planning Officers to develop personalised and creative support plans, enabling choice and control through personal budgets and Direct Payments.

• Collaborate with Community Development staff to link individuals with local community resources and services.

• Follow risk management and adult safeguarding procedures in all aspects of practice, while remaining mindful of potential children’s safeguarding concerns.

• Complete assessments and service user reviews professionally and efficiently, ensuring they accurately reflect the needs and outcomes of service users and carers.
