Exciting Opportunity for an Adults Social Worker in Hospital Discharge in Manchester! Earn £30 hourly in this ongoing role at Manchester Royal Infirmary Hospital. This full-time and fulfilling position within the Hospital Discharge Unit involves collaborative and impactful work with the Discharge Team on discharge planning and conducting Care Act 2014 Strengths Based Assessments for Pathway 2 and 3 care home placements. What you will do:
- Collaborate with the Discharge Team to plan, assess, and execute hospital discharges
- Conduct Strengths Based Assessments as per the Care Act 2014 for Pathway 2 and Pathway 3 care home placements
- Participate in Best Interest Meetings to aid discharge decision-making processes
- Handle Safeguarding cases and engage in Making Safeguarding Personal discussions related to Manchester Foundation Trust
- Manage Neighbourhood Apartment referrals and Crisis Clean referrals
- Work weekends on a rotational basis (1 in 7 weeks) with the Hospital Discharge to Assess team in Gorton
Requirements of the Social Worker:
- A degree or equivalent in Social Work
- Current registration with Social Work England
- Significant frontline experience in social services within the UK as a qualified Social Worker
