What you will do:

• Conduct strengths-based assessments of adults with care and support needs in accordance with the Social Services and Well-being (Wales) Act 2014.

• Develop, implement, and review person-centred care and support plans tailored to individual needs and outcomes.

• Manage a varied and complex caseload, ensuring timely interventions and effective risk management.

• Undertake Mental Capacity Assessments and contribute to Best Interest decision-making processes where appropriate.

• Respond to safeguarding concerns and undertake safeguarding duties in line with statutory guidance and council procedures.

• Work collaboratively with health professionals, care providers, housing services, and partner agencies to coordinate support.

• Arrange, commission, and monitor care packages, ensuring services remain appropriate, effective, and provide value for money.

• Undertake scheduled and unscheduled reviews of care and support arrangements, adapting plans where required.

• Maintain accurate, comprehensive, and timely case records using electronic case management systems.

• Prepare detailed reports for meetings, legal processes, safeguarding enquiries, and decision-making forums.

• Provide information, advice, advocacy, and signposting to adults, families, and carers.

• Participate in multidisciplinary meetings and contribute to integrated working practices.

• Ensure compliance with legislation, professional standards, local authority policies, and best practice guidance.

• Contribute to service improvement initiatives and promote innovative, strengths-based approaches across the team.

• Attend regular supervision, appraisal meetings, and mandatory training programmes.

Requirements:

• Degree or equivalent qualification in Social Work.

• Current Social Care Wales registration or eligibility to register.

• Experience working within Adult Social Care.

• Strong knowledge of the Social Services and Well-being (Wales) Act 2014, safeguarding procedures, Mental Capacity Act, and relevant legislation.

• Experience undertaking assessments, care planning, safeguarding, and complex case management.

• Excellent communication, analytical, and decision-making skills.

• Ability to work independently and collaboratively within multidisciplinary teams.
