What you will do

• Undertake comprehensive assessments, reviews, and support planning in line with the Care Act 2014.
• Manage a complex caseload of adults with learning disabilities, delivering timely and effective interventions.
• Carry out Mental Capacity Act assessments and make Best Interest decisions when required.
• Lead safeguarding enquiries under Section 42 to protect vulnerable adults.
• Work collaboratively with health professionals, service providers, families, and carers to deliver holistic care.
• Support service users to access appropriate resources, promoting independence and improving quality of life.
• Prepare detailed reports, attend multidisciplinary meetings, and contribute to case conferences.
• Maintain accurate and up‑to‑date case records in line with statutory and local authority requirements.
• Contribute to service development and continuous improvement initiatives within the LD Community Team.
