What you will do:

- Conduct comprehensive assessments of individuals’ activities of daily living, focusing on both strengths and needs
- Develop and implement person-centred and holistic intervention plans
- Collaborate effectively with a wide range of partners, professionals, and agencies to ensure best outcomes for clients
- Apply your knowledge of equipment and adaptation solutions to meet individual needs
- Manage and prioritise a dynamic caseload efficiently
- Maintain accurate records of interventions and assessments using the Mosaic recording system
- Commit to continuous professional development and reflective practice within the framework of relevant codes of conduct
- Incorporate safeguarding principles into everyday practice
- Travel to make home visits as required to provide on-the-ground support

Requirements:

- Current registration with the Health and Care Professions Council (HCPC)
- Minimum of 2 years’ experience in Occupational Therapy
- A valid driver’s licence is preferable for travel to client locations
