This vibrant role spans across health, social care, SEND, and education, focusing on preparing young people aged 14 to 25 for adulthood. Your day-to-day will involve carrying out Care Act assessments and drafting care and support plans, alongside a raft of other engaging duties. What you will do:

• Carry out comprehensive Care Act assessments for young individuals preparing for adulthood.
• Develop detailed care and support plans tailored to the needs of each young person.
• Participate in Mental Capacity Act assessments and Best Interest meetings.
• Collaborate with colleagues to ensure a smooth transition for young people into adulthood.
• Engage with multi-disciplinary teams to foster independence for individuals with diverse needs.
• Support and advise families to ensure they are informed and prepared for changes.
