We have a requirement for qualified Social Workers who have experience of working in a community setting. There are opportunities to work with adults of all ages across a range of conditions. Experience of adult support and protection is preferred. We currently have a requirement for at least two full-time equivalent Social Workers for up to six months. The working hours are negotiable with a general schedule of Monday to Friday, 9 am to 5 pm.

What you will do:

  • Conduct comprehensive assessments of adults in the community to determine their care needs.
  • Develop and implement care plans tailored to each individual's requirements.
  • Coordinate with a multidisciplinary team to ensure holistic care and support for clients.
  • Monitor and review client progress, making adjustments to care plans as necessary.
  • Provide support and protection for vulnerable adults, ensuring their safety and well-being.
  • Liaise with family members, carers, and external agencies to facilitate comprehensive care networks.
