What you will do:
- Maintain pharmacy services at Welney Ward, providing essential support to clinical pharmacy services, notably in specialties like rehabilitation and palliative care.
- Conduct medicines reconciliation and assess patient medicines upon admission, resolving issues and documenting these accurately.
- Manage medication orders for patients and ward stock.
- Participate in weekly MDT meetings to support patient care and discharge preparation.
- Assist patients and nursing staff with self-administration of medicines, promoting rehabilitation and independence.
- Liaise with community pharmacies during patient admission and discharge when required.
- Provide vital pharmacy technician support across CPFT bases in Cambridge.
Successful candidates will hold GPhc registration with at least two years of experience in specialised pharmacy services. While having a driving license is preferable, the role prioritises commitment to patient care and enhancing pharmacy service delivery.
