Key Responsibilities

• Review care and long-term support plans for vulnerable adults in line with the Care Act 2014.

• Collaborate with families, other professionals, and community resources to create inclusive and effective care plans.

• Safeguard adults from abuse, neglect, or exploitation through proactive and timely interventions.

• Work autonomously within legal and ethical frameworks, maintaining high professional standards.

• Manage a complex caseload, carrying out assessments, reviews, and support planning as required.

• Maintain accurate records, ensuring clear communication and documentation of all actions.

• Promote equality, diversity, and inclusion in all aspects of your work.
