What You Will Do

• Carry out comprehensive assessments of patients in the community, identifying needs and developing personalised care plans.

• Work closely with a multidisciplinary team to ensure coordinated, high-quality support and improved outcomes for patients.

• Deliver evidence-based interventions designed to promote independence and reduce reliance on formal care services.

• Monitor and review progress, adjusting care plans as needed to ensure the best possible results.

• Provide advice and education to patients and families, empowering them to manage their health and wellbeing.

Requirements

• HCPC registration as an Occupational Therapist.

• Experience in community-based practice, ideally within Adult Social Care.

• Valid driver’s licence to facilitate travel within the Liskeard community.
