What you will do:
- Conduct assessments and reviews using strength-based approaches to support young adults transitioning to independence.
- Collaborate with a range of agencies including children's services, health, voluntary organisations, and education providers to ensure holistic support.
- Develop personalised support plans, ensuring they reflect the individual's needs and goals, and explore all available resources and networks.
- Facilitate reablement services and the use of assistive technology to enhance independence and reduce risks.
- Manage and prioritise a caseload, making assessments and interventions where required, including face-to-face visits and telephone enquiries.
- Apply legal and professional knowledge, conducting safeguarding enquiries and promoting self-management among individuals at risk.
- Maintain professional and accurate records, contributing to reports for legal and service development purposes.
- Engage in continuous professional development to continuously hone your skills and deliver the best outcomes.
