As the front door to adult social care services, you'll play a crucial role in clearing a backlog of new referrals, expertly identifying when reablement or community resources can support independence, and conducting comprehensive Care Act assessments to determine eligible needs and the best options to meet them.
This fast-paced role requires a confident Social Worker adept in defensible decision-making, working autonomously, and efficiently managing a high turnover with shorter term work assignments. There is an expectation to conclude tasks promptly and to transfer ongoing work to other teams as needed, with weekly allocations to manage.

What You Will Do:

Evaluate and process new referrals to adult social care, determining necessary interventions to support service users.

Identify opportunities for reablement or community resources to maximise service user independence.

Conduct Care Act assessments to ascertain eligible needs and appropriate response plans.

Confidently make defensible decisions in a fast-paced, high-pressure environment.

Conclude work assignments in a timely manner and transfer ongoing cases as appropriate.

Manage weekly allocations to meet team goals and service user requirements.
