What you will do:

• Provide a professional Occupational Therapy and Reablement Service, tackling complex cases to help individuals achieve the best possible outcomes.

• Conduct strength-based and person-centred assessments to deliver tailored interventions.

• Assess and provide necessary equipment and adaptations to maximise independence and optimise the quality of life for individuals and their carers.

• Work in collaboration with health colleagues to ensure timely hospital discharges and support community integration.

• Communicate effectively with clients, carers, and various professionals to ensure a coordinated approach to care.

• Manage your caseload efficiently and maintain thorough records in line with Social Care requirements.

• Engage in continuous professional development and contribute to service improvement efforts.
