What you will do:

• Conduct backlog assessments and comprehensive care planning for patients recently discharged from the hospital.

• Attend daily in-person meetings for the first few weeks to establish rapport and ensure efficient operation processes, transitioning to up to two days of remote work as you settle into your role.

• Collaborate effectively with healthcare professionals to ensure seamless and efficient discharge planning.

• Engage with patients, their families and support networks to ensure they are fully prepared and supported as they transition from hospital to home.

• Maintain accurate records and documentation as required by hospital policies and professional standards.
