Key Responsibilities

Conduct comprehensive assessments in accordance with the Care Act.

Carry out S9 assessments and S27 reviews.

Support S117 Aftercare responsibilities.

Fulfil duties in line with the Mental Capacity Act 2005, Mental Health Act 1983/2007, and the Equality Act 2010.

Work in partnership with agencies and multidisciplinary teams to deliver holistic and effective care for service users.
