What you will do:

  • Facilitate multi-disciplinary assessments and reviews for children and young people in line with standard operating procedures and policies.
  • Maintain effective relationships with internal and external stakeholders, such as social care and educational services.
  • Participate in transition meetings for young people aged 16-25 to ensure seamless care transitions.
  • Support joint commissioning initiatives for special educational needs and disabilities (SEND), focusing on holistic care plans.
  • Manage and implement ongoing complex case management, including Personal Health Budgets and safeguarding procedures.
  • Collaborate with healthcare professionals to conduct comprehensive assessments of complex cases.
  • Ensure compliance with quality standards, policies, and the National Framework, enhancing safety for children and young people.
  • Participate in data management and reporting to contribute to informed decision-making processes.
  • Conduct investigations into continuing care eligibility and support resolution of any disputes.
