What you will do:
- Conduct thorough assessments and implement effective treatment plans tailored to patients within acute, inpatient, and trauma wards, focusing largely on an elderly caseload.
- Collaborate with a multidisciplinary team to plan and facilitate patient discharge processes.
- Provide guidance and support to patients and their families, enhancing their rehabilitation journey.
- Document patient progress and outcomes effectively to ensure continuity of care.
Requirements:
- Hold registration with the HCPC.
- Minimum of 2 years of experience in a similar setting is essential, providing you with the skills to tackle this exciting role head-on.
