What you will do:
- Conduct comprehensive assessments both at clients' homes and bedside in the hospital to facilitate smooth and effective discharges.
- Utilise your expertise to promote independence and support clients on their reablement journey, helping them regain their quality of life.
- Complete Care Act assessments by evaluating the ongoing needs of individuals, ensuring their personalised care plans are executed successfully.
- Collaborate with a multidisciplinary team to develop and implement tailored occupational therapy interventions.
- Travel as required to carry out home assessments, so having your own transport is necessary.
- Attend occasional office meetings to ensure cohesive team communication and strategic planning.

Requirements:
- A degree or equivalent in Occupational Therapy.
- Recent UK experience working as an Occupational Therapist, with a solid understanding of NHS systems.
- HCPC registration is essential.
- Must have at least two years post-qualification experience.
- Your own transport is a must for conducting necessary fieldwork.
