What you will do:

- Collaborate in accordance with the Care Act 2014 to provide proactive support and assess the needs of clients and their carers, ensuring access to the right services, especially focusing on health, safety, and well-being.
- Embrace and implement a Strength-Based Approach to deliver person-centred services that respect individual dignity, cultural and communication requirements, offering choices aligned with personal aspirations.
- Forge strong partnerships with various support services, including Technology Enabled Care, the Review Team, Re-ablement initiatives, and others to provide a cohesive service delivery.
- Innovate with creative solutions that bridge Adult Social Care and Health, enhancing the relationship with service users and stakeholders.
- Identify risks and lead investigations into potential abuse or neglect cases, creating and implementing protection plans and chairing safeguarding meetings as necessary.
- Coordinate and chair multi-disciplinary meetings to sustain effective collaboration.
- Maintain electronic case records accurately, in line with Council procedures and statutory demands, ensuring transparency and accountability.
- Supervise, guide, and manage staff, ensuring optimal performance and development.
- Support the Team Managers and Service Manager with operational issues, safeguarding both staff and clients’ welfare and contributing to a safe service.
- Engage in other tasks aligned with specialist skills and current policies, offering a flexible and rewarding work environment.
