We are seeking experienced and compassionate Occupational Therapists to join the Lifelong Services Reviewing Team in West Sussex. Key Responsibilities:

• Complete strengths-based reviews for adults aged 18+ with physical disabilities, learning disabilities, acquired brain injuries, and long-term health conditions.

• Undertake Mental Capacity Act assessments and complete Continuing Health Care checklists and decision support tools.

• Collaborate with a multi-disciplinary team to create tailored, evidence-informed support plans.

• Deliver risk assessments, equipment provision, and recommendations for home adaptations to promote independence.

• Provide specialist occupational therapy input to contribute to holistic assessments and care planning.

• Travel across the West Sussex area to conduct community-based assessments, promoting independence and quality of life.

Requirements:

• HCPC registration as an Occupational Therapist is essential.

• A valid UK driving licence is preferred due to the nature of community visits.

• Experience in social care or community-based occupational therapy is desirable.
