We are on the lookout for a dynamic Adult Social Worker to join our Head of Early Help and Prevention First Response Team. What you will do:

- Provide strategic and operational leadership for First Response and Prevention services in adult social care.
- Oversee intake and screening at the front door, early intervention, prevention, enablement, and reablement pathways to promote independence and reduce dependency on long-term services.
- Ensure service delivery aligns with statutory frameworks such as the Care Act and CQC standards, integrating closely with health services, third sector organisations, and community teams.
- Lead the development of a unified front door approach, melding screening, prevention, and enablement into a seamless service.
- Drive innovation and transform service models to focus on strengths-based, outcome-oriented practice.
- Oversee adult social care teams involved in intake, prevention, reablement, discharge, and enablement.
- Manage service performance, budgets, compliance, and quality assurance in line with CQC and Care Act standards.
- Ensure effective risk management, safeguarding of vulnerable adults, and continuous team performance oversight.
- Foster partnerships and collaborations with NHS, community health services, the third sector, and housing for coordinated care pathways.
- Champion a system leadership approach that aligns with ICS and locality-based care priorities.
- Lead diverse multidisciplinary teams, providing supervision and supporting a culture of learning and innovation.
