This position offers options for full-time or part-time employment, and as we operate a 7-day service, your work may include 35 hours a week with some weekends.

What you will do:

  • Collaborate with the multidisciplinary team to ensure thorough and holistic assessments for patients being discharged from the hospital.
  • Facilitate smooth transitions for patients from the hospital to community settings, ensuring all health and social care needs are addressed.
  • Conduct comprehensive assessments in the community, focusing on empowering individuals through a strengths-based approach.
  • Provide expert guidance and support to patients and families during the discharge process, offering information and resources to support informed decision-making.
  • Work flexibly across various services, including Crisis response, intermediate care, and discharge teams within the hospital setting.


The ideal candidate:

Must be a registered Social Worker with Social Work England and have experience working in a fast-paced environment.
