What you will do:

- Conduct comprehensive assessments of children and families to identify needs and develop action plans
- Provide tailored support and intervention to children and families, enhancing their social, emotional, and physical well-being
- Collaborate closely with multi-agency teams to deliver coordinated and effective support
- Ensure all case records are maintained accurately and in a timely manner in accordance with statutory requirements
- Participate actively in team meetings and contribute to service development initiatives
- Liaise with other professionals such as health workers, teachers, and police to ensure a holistic approach to social care
