Social Worker - Leaving Care Team in Swindon (UK) - £38.76 Hourly - Ongoing - Full Time

This is an exciting opportunity for a Social Worker within the Leaving Care Team in Swindon! Here, you will embark on a rewarding journey, supporting young people as they transition from care to independence. In this full-time role, you will manage a caseload of young individuals preparing to leave care, while collaborating closely with dedicated Pathway Advisors to develop and maintain comprehensive pathway plans. As part of our vibrant team, you will also participate in our duty system, offering crucial support to young people up to the age of 18.

What you will do:

- Manage a caseload of young people preparing to leave care and transition to independence.
- Work alongside Pathway Advisors to develop, complete, and maintain pathway plan assessments and initial pathway plans.
- Ensure compliance with required legislation and maintain plans until the young person turns 18.
- Participate in the duty system, providing additional support to young people when necessary.
- Collaborate with other services to ensure seamless support for young people during their transition.
