What you will do:
- Conduct specialist needs assessments, such as Single Assessments and Children in Need evaluations, ensuring comprehensive and child-centred care plans are developed.
- Prepare detailed care and support plans, addressing a wide range of needs including health, education, transitions, respite, and family support.
- Regularly review care packages, budgets, personal/direct payments, and transitions, ensuring all arrangements meet the current needs of the child.
- Coordinate effectively with multi-agency partners, including education and health services, ensuring cohesive and integrated support for each child.
- Actively participate in meetings such as Team Around the Child and EHCP reviews to ensure excellent communication and planning.
- Engage directly with children using suitable communication techniques, advocating strongly for their needs and the voices of their carers.
- Support families through safeguarding and legal processes, ensuring compliance with statutory duties including the Children Act 1989 and the Equality Act 2010.
