Prison Link Worker - Hull (UK) - £24,000 Annually - Full-time, Permanent
Who we are looking for:
We are seeking a dynamic individual to join our team as a Prison Link Worker in Hull, primarily serving HMP Humber and HMP Hull. This role is part of our innovative Dependency and Recovery Service, which is renowned for supporting individuals in transforming their lives for over 25 years. You will play a pivotal role in supporting men within the prison establishment before and after their release, ensuring a seamless transition back into the community. What you will do:
- Engage with prisoners prior to their release to build strong relationships, facilitating maximum engagement post-release.
- Develop internal pathways with prison substance misuse teams and establish a robust four-week community post-release plan.
- Conduct comprehensive risk and needs assessments, develop SMART action plans, and deliver regular interventions.
- Meet individuals on release day to provide immediate support, linking them with relevant community resources and appointments.
- Deliver one-on-one sessions and facilitate group work as per action plans.
- Maintain accurate records and data on the Refer and Monitor system, ensuring all interactions and changes in circumstances are documented.
- Collaborate effectively with key partners, including substance misuse services and probation staff, to ensure comprehensive support for service users.
- Promote our services at meetings and events, and provide essential addiction education and harm reduction information.
- Comply with all targets, performance standards, and policies set out in your role.
