What you will do:

• Conduct thorough occupational therapy assessments to establish patient needs and plan accordingly.
• Implement effective treatment plans aimed at facilitating patient discharge and supporting community integration.
• Manage and coordinate discharge to assess and supported discharge pathways, ensuring smooth transitional care for patients.
• Collaborate with multidisciplinary teams to enhance the quality of care and overcome any challenges in implementing therapy interventions.
• Maintain detailed documentation and records in accordance with HCPC standards, ensuring compliance and quality care.
