What you will do:

Conduct assessments to identify the specific needs of clients with a focus on major home adaptations and equipment.

Design and recommend effective adaptation solutions to improve accessibility and promote independence.

Work collaboratively within a multidisciplinary team to ensure the smooth delivery of adaptation plans.

Offer clear guidance and expert advice to clients, families, and professionals on recommended solutions.

Maintain detailed and accurate records of all assessments, plans, and progress.
Requirements:

HCPC registered Occupational Therapist

Previous experience in major adaptations preferred

Driving licence advantageous for efficient travel during home visits
