Social Worker - Older Persons Team - Leicester, £34 per hour, Hourly, Ongoing contract

You'll also be providing longer-term support to those receiving adult social care living in the west of the city or whose GP is part of the primary care network in the West. What you will do:
- Perform comprehensive assessments of older adults and individuals with physical health conditions.
- Coordinate and support the discharge planning process for older adults from mental health wards.
- Provide ongoing care and support management for adults in receipt of social care.
- Collaborate with healthcare providers in the West area of the city to ensure cohesive primary care support.
- Attend face-to-face team meetings every four weeks to discuss strategies and share insights.
- Adapt supervision to match both your needs and those of your clients, promoting a flexible support structure.

Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant frontline experience in Social Services within the UK as a qualified Social Worker
