What you will do:

  • Conduct complex, holistic assessments and develop person-centred care and support plans using a strengths-based model.
  • Apply expert understanding of the legal social care framework, ensuring all actions comply with relevant legislation including the Care Act, Mental Capacity Act, and Mental Health Act.
  • Focus on helping individuals maintain independence in their homes and community by leveraging local resources and personal networks.
  • Deliver services that empower people to optimise their abilities and enhance their quality of life through whole family approaches.
  • Collaborate seamlessly within a multi-disciplinary team to ensure holistic care and support.


Requirements:

  • Must be registered with the Health and Care Professions Council (HCPC).
  • A valid driving licence is essential to visit clients across Redbridge.
