What you will do:
- Conduct clinical screenings of medication charts on the ward to ensure safe and effective patient care.
- Review and screen discharge prescriptions and one-stop orders, maintaining high standards of pharmaceutical practice.
- Complete medicines reconciliation and address ward-based medication information queries.
- Engage in multidisciplinary team (MDT) working to contribute to comprehensive patient care plans.
- Perform clinical checking and ward CD audits, along with managing medication orders.
- Collaborate closely with Medicines Optimisation Technicians to provide optimal pharmaceutical services.
- Interpret blood results in relation to medication needs, offering insight and recommendations for treatment plans.
Requirements:
- Must be a registered pharmacist with the Health and Care Professions Council (HCPC).
- A valid UK driving licence is required, as this role may involve travel between sites.
