What you will do:
- Collaborate within a multi-disciplinary team to ensure seamless hospital discharges and support plans.
- Conduct comprehensive care and support assessments tailored to individual needs.
- Undertake mental capacity and Best Interest's assessments to safeguard client interests.
- Engage in vital safeguarding work to protect vulnerable individuals.
- Work with the transfer of care hub, prioritising and triaging for efficient patient support.
- Develop and review reablement plans to aid in the recovery and rehabilitation process.
- Provide essential support to carers, recognising their crucial role in the care process.
- Participate actively in decision support tool meetings to ensure comprehensive care planning.

Requirements of the Social Worker:
- A degree or equivalent qualification in social work is essential.
- Current registration with Social Work England is required.
- Significant frontline experience within UK Social Services is necessary, demonstrating your competence as a qualified Social Worker.
